Community Outreach & Engagement Team
The City of La Habra employs two full-time Community Outreach Liaisons, providing daily team coverage to engage with individuals who are experiencing homelessness in La Habra. Assessments are made using a standardized assessment process as a road map to customize the needs of the individual and their unique circumstances and eventually match them with further supportive services conducted by various regional partners. The priority is to provide a consistent transparent process for matching individuals and families to the most appropriate linkages to services and housing resources through the County's Coordinated Entry System. Referrals include a warm hand-off between access point providers and housing or supportive service providers. These supportive services and resources include shelter, mental health assistance, military veterans' support, transitional programs and substance abuse care. The La Habra Community Outreach & Engagement Team responds to community inquiries regarding areas of concern within the City.

Community Outreach Program
The Community Outreach Program began in May 2022. Since then, we have had 1,786 client engagements, placed 79 clients into emergency housing, placed 16 clients into permanent housing, placed 19 clients into transitional housing, residential back to work programs, and detox centers, and placed three military veterans into Villages, Cabrillo and VOALA housing.
We also partnered with the Orange County Healthcare Agency, Pathways of Hope, HIS-OC, Family Promise, HOPE Center, Volunteers of America (VOA), PATH, Mercy House, Family Solutions Collaborative and Individual Coordinated Entry System, Transitional Aged Youth Coordinate Entry (TAY) and Veterans Coordinated Entry, O.C. Public Health Nurses, Hillcrest Health & Wellness Center, and Lutheran Social Services.
